An overview of nursing case management scope and standards of practice, exposure to important information related to health policy including insurance changes as outlined in the Affordable Care Act, emphasis on concepts of patient engagement and activation including health literacy and behavior change strategies, the role of health information technology in care coordination, and concepts of collaborative team based leadership are discussed. The triple aim of quality, safety and cost containment are emphasized throughout the program. An opportunity to examine practice application including strategies for effective home visiting, medication reconciliation, communication and goal setting, and telephone case management are provided.
Following the face to face intensive, a series of three on-line self-study modules are included to further augment your learning. Upon completion of the program, participants will receive a certificate of program completion.
This program is offered in a hybrid format with a 3 day face to face intensive training followed by a series of three online self-study learning modules.
Registered Nurses and Advanced Practice RNs who are new, experienced or certified in the case manager role / care coordinator role; Supervisors and Program Developers for RN Case Manager/Care Coordinator positions.
$1200.00 per student includes the three day didactic seminar, three case study and practice model learning modules teamed with facilitated teleconference discussion, guidelines for clinical residency including competency/skills checklist, and recommended practice tools and resources.
